Child Placement Contract

A. About My Place (just for kids) inc.

  1. We are a licensed child care program that operates as a drop off child care center.
  2. Our Staff is carefully selected and trained to ensure that your child will receive the best care.
  3. There will always be two staff members in the building, one of which, if not both, will be CPR and First Aid certified.
  4. Our center allows for children to learn and grow through a play-based structure, which also includes staff guidance. Parents are encouraged to call or visit any time to observe their child at MY PLACE (just for kids) inc.
  5. By law, we must follow a ratio of 1 adult per 4 children for ages 2 months to 30 months, 1 adult per 8 children for ages 2 1/2 to 3 years and 1 adult per 10 children for ages 3 to 12 years.
  6. You must sign a child in sheet stating that your child is in good health and that you have read this contract before leaving the building each time you visit.

B. HEALTH

  1. Child information cards must be completely filled out by parents or guardians before or on the first visit. Please update these cards when there is a change of address, phone number, doctor, or substitute guardian.
  2. If health conditions warrant special care, or if your child is left for extended care, obtain a green health and have a doctor fill it out. Please submit this to us, so that we have it on file.
  3. Notify the center if your child develops a communicable disease (i.e. mumps, chicken pox, etc.) and was recently at or will be visiting our center.
  4. IF YOU ARE IN ANY DOUBT ABOUT YOUR CHILD'S HEALTH, OR IF A FRESH COLD IS PRESENT, PLEASE KEEP YOUR CHILD AT HOME. If an illness or symptoms of an illness are detected after check-in, your child will be isolated and parents will contacted for an immediate pick-up. (within 30 minutes).
  5. Please inform MY PLACE of any diet restrictions or medical conditions at time of drop off, before you leave the center. For your convenience we will place this on permanent file.

C. General Program

  1. Your child will be offered arts and crafts, computer games, quiet time as well as social interaction with staff and children.
  2. We will provide indoor play equipment and developmental toys that encourage self-expression.
  3. Children will be disciplined with positive methods, which will encourage self-control, self-direction and cooperation. (In extreme cases time outs will be used and parents may be called.) Children will be praised for good behavior.
  4. Fighting, biting or extreme misbehavior will not be tolerated.
  5. In case of any accident we will first try to notify the person that is left as a contact person at the time of check in. If need be we will contact those listed on the emergency card and lastly the child's physician.
  6. All children present for five hours or longer will be allowed to go outside for a walk under careful staff supervision.

D. MEALS

  1. Meals or snacks must be provided by the parents, or may be ordered and purchased in advance from participating local restaurant and will be delivered at meal time. My Place can not provide additional food or drinks other than that which was sent or arranged for the child.
  2. You may also leave snack money for use in our machines on site. Please specify any restrictions.
  3. Due to health regulations, all food that has been opened must be thrown away if not eaten. (i.e. partially eaten fruit or sandwich, etc.)
  4. All infant formula and food must be pre-made, mixed and labeled with the child's name and contents as required by child care licensing. You must supply all formula in plastic, non-breakable containers. All formula will be stored properly until needed. any leftover product will be discarded if not finished.
  5. Due to Department of Health regulations, food cannot be heated or prepared by staff at MY PLACE.
  6. Notify MY PLACE at check-in time about any food allergies or special feeding instructions.
  7. By health department law, all children and staff must wash their hands before handling and eating food.

E. FOR CHILDREN 2 MONTHS TO 30 MONTHS

  1. The infant activity area is specially designed for the needs of infants and toddlers, offering a variety of age appropriate toys.
  2. If conditions permit, children will be allowed full use of the center with careful supervision.
  3. Infants 12 months and under will have a flexible schedule allowing them to eat and nap on demand unless specified by parent.
  4. Please call in advance to reserve a spot for your child. (Under 30 months) Please be fair and only stay the time that you reserve.
  5. Please call 24 hours in advance if you need to cancel an appointment, failure to do so will result in a $10.00 fee.
  6. Last minute reservations or walk-ins will be accepeted if space permits, but is not guaranteed.

G. DIAPERS AND TOILET TRAINING

  1. Please supply diapers, wipes and change of clothes. Diapers will be checked hourly or as needed.
  2. If your child is toilet training, please leave instructions at time of check in. We will work with you to help the process along.
  3. According to health regulations staff must wash hands before and after diaper changes. Children and staff must wash their hands after using the bathroom.
  4. My Place will supply diapers and clothes for a charge if not provided by the parents. A $5.00 fee is charged for use of My Place clothes. This fee will be refunded if the clothes are returned within 30 days. A $2.00 fee is charged for each diaper used that is not supplied by the parent.
  5. Any clothes, toys or equipment donations will be greatly appreciated.

H. NAPS/SLEEPING

  1. Cribs and cots are available for sleeping.
  2. children ages 3 and under will be given quiet time so that sleep and relaxation can occur. Please provide a favorite blanket or pillow. If you give a specific time we will follow that; however, please be advised that by law we must enforce a quiet time for all children of this age.
  3. Infants will be given rest time as needed.
  4. For late night stays, we would be happy to put your children into their P.J.'s. Children will be given a quiet time or bedtime as specified by parents. Older children will sleep upon self-initiation unless specified.
